The following survey has been developed to understand the experience of Canadian health care professionals in relation to children’s environmental health. The data collected through this survey will assist in the development of capacity-building workshops for Canadian health care professionals. The survey is individual, voluntary and confidential. It was asked that each question be answered based on each practitioner’s own experience.The word “children” is used throughout to represent girls and boys from infancy to adolescence. The estimated time to complete the survey is approximately 15 minutes. This survey has been adapted from a survey developed by the Canadian Institute of Child Health and the Asociación Argentina de Médicos por el Medio Ambiente for their project “Measuring the Environmental Impact on Children’s Health in the Southern Cone” funded by the Canadian International Development Agency.
